Custom paper size not working?

I made an album with screenshots and descriptions: http://imgur.com/a/TzSAq

Basically, setting a custom paper size does not seem to work right.  The paper appears to be many times larger than it is supposed to, i.e. a 10" circle is a tiny thing in the corner of a 12x18 page, where it should take up most of the height.

Am I missing something totally obvious here?
